Assessment Zip File

This download includes clinician scoring tools developed by Piper Harris, LPC, to streamline assessment scoring and interpretation. The scoring forms are formatted for use in Excel or Google Sheets and automate calculations, scoring bands, and clinical summaries.
 
For convenience, the zip file also contains publicly available PDF versions of the assessment instruments referenced in the scoring tools. These assessment instruments are the intellectual property of their respective authors and publishers and are included solely to allow clinicians to administer the measures referenced in the scoring sheets.
 
Purchase of this product grants access to the scoring systems and workflow tools created for clinical efficiency. The assessments themselves are not being sold, licensed, or transferred, and users are responsible for ensuring they have appropriate permission to use any assessment in their clinical practice.
